MFPP Renewals

The eligibility period for the Minnesota Family Planning Program (MFPP) is 12 months. This period begins on the first day of the month of application and ends on the last day of the twelfth month. The eligibility period is set by the month of application, regardless of whether the person received presumptive eligibility. Enrollees must submit the Minnesota Family Planning Program Renewal (DHS-5440) and complete the renewal process before the end of the eligibility period.
